1. Go to MyHSHS registration page on https://www.myhshs.org/MyHSHS/app/activation
2. Click on the “Sign Up with your information”
Complete the following steps to sign up for a MyChart account to access your MyHSHS portal.
Enter your personal information.
Verify your contact information.
Choose a username and password.

3. Enter your the following information;
First Name, Middle Name and Last Name
Address including “Country”, Street, Apartment or Unit, City, State
4. Other information
You will also need to provide the following information;
* Date of birth
* Social Security Number
* Legal Sex
* Home phone number
* Email address.
5. Click on “Next” and complete the MyHSHS sign up for your MyChart account
Once your account has been completed, make sure to keep your MyChart login details safe as you will always need them to login MyHSHS account. For MyHSHS login via MyChart MYHSHS patient portal, you will need your Username and your password.
